Form can be downloaded and then emailed to Loscam Customer Service or printed and sent via fax or mail.

For transfer dockets with date corrections that are over 2 months old the Back Dated Correction Authority will need to be filled in and signed by the receiving parties. Please forward both the transfer docket (s) and signed Back Dated Correction Authority form to Loscam Customer Service for processing.
