For transfer dockets that are over 2 months old the BDA will need to be filled in and signed by both the receiving and sending parties. Please forward both the transfer docket (s) and signed BDA form to Loscam Customer Service for processing.
For transfer dockets with date corrections that are over 2 months old the Back Dated Correction Authority will need to be filled in and signed by the receiving parties. Please forward both the transfer docket (s) and signed Back Dated Correction Authority form to Loscam Customer Service for processing.
